Two employees were adjusting forks on a forklift before lifting a load of building materials at a distribution warehouse. While attempting to spread the forks apart, one of the forks fell off of the tracks. Because the fork was no longer seated in the tracks, it fell to the ground, striking one of the employees. On or about 3/31/2015, at approximately 9:50 p.m., an employee was hospitalized with a chin laceration. The employee was operating a mechanically powered hot trim press. After the press cycle, the work piece became stuck in the upper die of the press.
