As he was turning to exit the freezer, his glove became caught in the steering wheel and he lost control of the forklift. He was thrown from the operators' platform and became pinned between the forklift and a bollard post located at the freezer door opening, fracturing his pelvis.

An employee was picking up cut brush and placing it into the chipper during power line right of way clearance. The branch brushed his abdomen and lodged a thorn into his abdomen. The event occurred on Sept. 22, 2015. Several days and two doctor visits later, the employee was admitted to the hospital on Sept. 29, 2015 with a severe infection. The thorn was found during exploratory surgery.

An employee was cutting a limb on the ground using a chainsaw. The tip of the saw popped out of the limb and struck the employee's left foot behind the big toe, causing a laceration.

A maintenance employee was assisting another maintenance employee in repairing/replacing a belt on a dryer fan. He was checking the tension of the belt while the machine was slowing down after being shut down and caught his left hand in the pulley amputating the left thumb tip approximately half way down into the nail.

An AME palletizing machine had been installed to palletize bottles/stack them on plastic trays. An employee noticed that the machine had trays that were sideways. He opened the gate and entered the machine to straighten the trays. The crane inside the machine activated while he was inside, striking him and crushing him against the side of the machine.

An employee stepped over a pallet (used to store warehouse equipment) on the warehouse floor to retrieve some carpet squares. He picked up the carpet squares and stepped backwards to go back over the pallet. After doing so, he lost his balance and caught his toe on the pallet, which caused him to trip and fall. He landed on his knee and face, suffering an unspecified knee injury and a concussion. He was hospitalized.

Two employees were adjusting forks on a forklift before lifting a load of building materials at a distribution warehouse. While attempting to spread the forks apart, one of the forks fell off of the tracks. Because the fork was no longer seated in the tracks, it fell to the ground, striking one of the employees. The employee sustained a broken leg (fibula) approximately 2 to 3 inches above his right ankle and five broken toes on his left foot.

On or about 3/31/2015, at approximately 9:50 p.m., an employee was hospitalized with a chin laceration. The employee was operating a mechanically powered hot trim press. After the press cycle, the work piece became stuck in the upper die of the press. The work piece then fell from the die and struck the hand tongs the employee was holding, which caused the tong's handle to strike the employee in the chin.

An employee passed out and was found unconscious. The employee regained consciousness and was found to have a gash near the right temple. Blood was observed on a parts cart and the floor where the employee was first found. EMS responded and took the employee to Doctor's West Hospital. A cat scan was performed and the employee was found to have minor internal brain bleeding. The employee was taken to Grant Hospital for further tests.
