An employee was diagnosing an issue on a destoner. The employee went up a set of stairs and climbed onto the equipment. To get down, the employee jumped down onto the 180-degree pre-heat water reservoir cover. When the employee jumped down, his right leg landed on a carrot, which caused part of his right leg to go into a 3-by-3-foot open hatch and into the water below. The employee suffered second degree burns to his lower right leg.

On August 31, 2020, an iron worker was installing a steel wall curtain while attached to the steel wall curtain approximately 12 feet from the ground. The steel wall curtain was on an overhead crane and employees were making ties at certain points. When the steel wall curtain was released by the crane it overturned and the iron worker was either struck by the steel wall curtain and/or struck an air compressor that was on the ground where he fell. The employee was hospitalized for fractured ribs.

On August 11, 2020, an employee was working in a textile mill operating a fabric line in the pre-shear department. The fabric was beginning to curl under prior to entering the shear. The employee attempted to uncurl the fabric and contacted the spinning shear blade resulting in amputation of their left index and middle fingers above the first joint. The shear blade was not guarded at the time.

An employee was clearing a jammed part in a tapper machine. The employee's right index fingertip got caught in the jaws of the tapper machine as they closed. When the employee pulled their finger back, their fingertip remained in the jaws and was avulsed, and the bone of the fingertip was fractured. At the time of the incident, the machine was in manual mode.

While he was riding the lift gate up and demonstrating this process to a trainee, he tried to reposition his feet, shifting from one foot to the other.
