An employee was diagnosing an issue on a destoner. The employee went up a set of stairs and climbed onto the equipment. To get down, the employee jumped down onto the 180-degree pre-heat water reservoir cover. When the employee jumped down, his right leg landed on a carrot, which caused part of his right leg to go into a 3-by-3-foot open hatch and into the water below. The employee suffered second degree burns to his lower right leg.

On August 31, 2020, an iron worker was installing a steel wall curtain while attached to the steel wall curtain approximately 12 feet from the ground. The steel wall curtain was on an overhead crane and employees were making ties at certain points. When the steel wall curtain was released by the crane it overturned and the iron worker was either struck by the steel wall curtain and/or struck an air compressor that was on the ground where he fell. The employee was hospitalized for fractured ribs.
