On March 7, 2016, an employee was moving a motor weighing approximately 350 pounds when the motor broke the floor, creating a hole. The employee fell approximately 25 feet through the hole, dislocating the left shoulder, fracturing an elbow, breaking a hip, and possibly rupturing the kidney and spleen.
