An employee turned off a lathe machine, opened it, and used a push bar to push material into the lathe. The push bar was still in the machine when the lathe was reactivated. An employee was trying to transfer lumber from a bundle on a forklift into the bed of a truck. The lumber was frozen together, and another employee began to tap it with a hammer. The wood began to slide, and the first employee's hand was caught between the falling wood and a stack of wood. The employee's right ring fingertip was amputated.

At 7:30 a.m., an employee began work with an asphalt crack sealing team. At 2:30 p.m., he had begun to suffer severe leg cramps due to heat and dehydration. He was hospitalized.

An employee was walking to a linen cart to get towels when she lost her balance and fell, striking her head on the floor.

An employee had just started going down a ladder (+/- 4 rungs) when one of her feet slipped off a rung, causing her to lose her balance. She attempted to catch herself, but was not able to stop her momentum backwards. She hit the concrete floor below, suffering traumatic damage to multiple body parts, including but not limited to her head, neck and shoulder.

An employee was ascending a fixed ladder and opened a roof hatch. The roof hatch then slammed on his hand and broke his finger, causing him to fall approximately 8 feet, where he broke three vertebrae in his back.

An employee was working in a stairwell to conduct drywall finishing. The employee placed one end of a plank on the landing and the other end in the rung of an extension ladder and climbed onto the plank. The employee then fell 16 feet to the ground, fracturing the left heel and lumbar spine.

On July 9, 2015, an employee was providing paramedic services at the home of a patient. While attempting to lift the patient from the floor of a chicken coop, he slipped and fell on his posterior. He got up and continued working. Later, he reported increasing pain and bruising from his fall to his employer. He went to the emergency room and was admitted to the hospital for vitamin K therapy to control muscular bleeding.
