The injured employee was standing by a bag house that that was approximately 27 feet away from where a discharge pipe was discharging wood pellets. A fire came out of the pipe and back flashed through a cooler and back through the piping system to the bag house where an explosion occurred. On August 7, 2019, employees were diagnosing a malfunctioning grease pump that is used to grease truck joints and moving parts. When the injured employee lifted the pump, the cylinder located inside the pump fell and lacerated the employee's left ring finger which later resulted in amputation of the fingertip.

As he started removing the cap, hot water and steam released from that area, striking the employee. He sustained third degree burns to his left wrist, left arm, and left side torso.
