An employee was attempting to separate a hex socket from the sleeve of an adapter that was used to link a bore machine to an auger. The employee was supporting the hex socket with a finger while tapping the socket with a small hammer to unseat it from the sleeve. The socket gave way, amputating the employee's fingertip with a guillotine action. |
