An employee was cutting a banded pack of 1-inch thick lumber boards. Immediately after cutting the band, the uneven pile of lumber fell toward him and knocked him over. His middle fingertip became trapped between two of the boards, resulting in a fingertip amputation.

On 8/6/15, an employee fell in the kitchen. Employee stated that he/she did not slip on anything and the floor was not wet. The employee lost balance and fell. The employee suffered a broken hip and a torn MCL in the right knee.
