On July 6, 2021, at approximately 8:30 AM a semi-truck was delivering rebar to the site. As an employee went to the rear of the truck to help guide it, a loader grabbed one side of the rebar and caused the other side to bounce and strike the employee on his left leg. The employee suffered fractures to his tibia and fibula.

With the assistance of a sling swing the employee was loading a big gas valve into a truck with his bare hands. During the loading, the sling rotated thus rotating the valve, which caused the employee to pinch his finger between the sling and the valve resulting in the amputation of his thumb just below the fingernail.

An employee laid the dock plate down on a trailer bed to unload a trailer. A piece of wood was sticking out from the trailer wall, preventing the dock plate from laying all the way down. The employee used his foot to kick the wood out of the way, which caused his foot to become stuck under the dock plate. His ankle broke and started bleeding.

An employee was removing material from a warehouse using a carry deck (crane). As he backed out of the space, his elbow got caught between a pole and the carry deck, causing a laceration and nerve damage to the elbow. He was hospitalized for surgery.
