An employee had just moved the bed of a tow truck using a winch attached to an overhead crane. As he set the bed down on a cart, the winch detached, came back up toward the employee, and struck his left hand, pinching it against the crane hook. He suffered a partial amputation of the middle finger.

An employee was helping building maintenance install a new dock plate. While the employee was installing the dock plate by hand, the dock plate shifted and caught the employee's gloved hand, pinching it between the new dock plate and the concrete dock. As a result, the employee's right thumb tip was amputated.

An employee was positioning a main cylinder on the back of a truck. The cylinder has a hole in it that is to line up with a hole on the back of the truck. Once lined up, a pin is to be inserted to hold the cylinder in place. A pry bar is used to help keep the cylinder straight and prevent bowing. The employee used a finger to see if the holes lined up. The pry bar moved and the employee's finger was pinched in the opening where the pin should go. The employee sustained a fingertip injury that

A temporary employee was drilling holes on a sub frame while wearing gloves when the employee dropped a screw in the machine. He tried to pick up the screw with his left hand and the machine caught the glove pulling his hand into the machine amputating his left pinky fingertip.

The injured employee was wiping off a roll of steel with a rag and cleaner. The employee directed another employee to rotate the roll. As the roll rotated, it pinched the injured employee's right pinky finger between the rolls of steel. The fingertip had to be amputated. The machine was not locked out/tagged out at the time.

An employee was changing the rolls on the slitter and had the slitter roll hoisted with a crane. The roll was crooked/off balance on one side, which caused the sling strap to loosen. As the employee straightened the sling strap, the roll dropped down and struck his fingers between the sling strap and the roll. His left-hand ring and middle fingertips were surgically amputated.
