An employee was removing a three-inch diaphragm pump from a storage room in the accumulator house. After placing the diaphragm pump onto the ground outside the storage room, he tripped on a board protruding out from under the right side of the accumulator house and fell to the ground knocking the pump onto his right calf muscle and crushing it.

The elevator lifting a 30 foot long pipe at an oil well shifted causing the bottom of the pipe to strike an employee in the back. This employee then fell against a second employee pushing the tongs into his face and causing employee 2 to fall and strike his head against the guardrail. Employee 2 was knocked unconscious for several seconds and was hospitalized with a fracture of the bottom orbital bone below the left eye.

An employee was disconnecting an attachment from the boom of an excavator for a transmission crew. When the connecting pin was removed, the 100-pound auger motor he was working on fell and crushed the end of his left pinky finger. He suffered an amputation of the distal end/joint of that finger.

A roll was being lowered into the furnace. The bottom edge of the roll caught the inside of the donut ring, causing the roll to turn out of the rigging shoe. An employee reached for the roll to clean off a refractory that had fallen down, unaware that the roll was out of the rigging. The roll fell through the donut and crushed the employee's hand. The employee suffered an amputation to the first knuckle of the right index finger; the right-hand middle finger was also fractured.
