| An employee was strapping supply wood. The employee saw that the panel saw was not working properly and tried to fix the problem. The accumulator arm came in to catch the plywood panels while the X-lift was discharging the full load of plywood. The panels on the other side of the machine did not catch the accumulator arm, and the panels fell through and hit the X-lift.
