An employee was waiting at the gate that leads to the press loader. His right foot was underneath the gate and his right toe was hanging over the edge of the pit. When the press loader step indexed down, his right foot got caught between the press loader step and the edge of the pit resulting in a crushing injury to his right toe. An employee was strapping supply wood. The employee saw that the panel saw was not working properly and tried to fix the problem. The accumulator arm came in to catch the plywood panels while the X-lift was discharging the full load of plywood. The panels on the other side of the machine did not catch the accumulator arm, and the panels fell through and hit the X-lift.
