An employee was riding in a telehandler's basket, preparing to clean a duct. The forks of the telehandler caught on a beam. The telehandler then backed up. This caused the force on the forks to be released, which jostled the employee. The side of his body bumped into the basket railing and he suffered three broken ribs and a punctured lung. He was hospitalized.

An employee was removing a part from under a rotational mold cover, held by a chain hoist about 6-7 feet above the working surface. The chain slipped about 1 foot, after which the load brake activated; this caused the cover to bounce off of the chain hook, strike the employee, and then come to rest on top of him. He suffered multiple fractures.
