An employee was clearing a metal pallet that was stuck on a filler line when a sensor triggered and the filler actuated, pinning the employee between the machine and the base layer of the pallet. The employee suffered four broken vertebrae and a fractured sternum.

A temporary employee was helping other employees separate bins that were stacked together. When a bin was separated from the stack and slid forward, the employee's right ring finger was pinched between the lip of the bin he was pulling out and the bottom edge of the bin that had already been moved from the stack. The fingertip was crushed and lacerated, and had to be medically amputated.

An employee was rigging angle steel sections to be hoisted by a crane. A section was lifted by hand and pushed in order to free it from the rest of the bundle. While holding the section, gravel shifted under his feet and caused the employee to lose control. The steel pulled back to its original position and caught the employee's hand against the resting section of steel. He sustained a crushing injury which required amputation of the right middle and ring fingertips.

On March 1, 2018, at 2:30 p.m., an employee was entering the break room, which had a newly surfaced floor, when she slipped and fell, fracturing her hip and requiring hospitalization. Her boots were wet at the time of the incident.

The employee was loading parts onto a rotating conveyor system when the machine malfunctioned and the parts got wedged in the machine. When the employee reached into the running machine to remove the parts, his finger was pinched resulting in the amputation of the left index finger to the first knuckle.
