On January 17, 2018, at approximately 3:00 p.m., an employee was picking up ventilation duct work on the outside retaining wall of an outdoor grain pile. A second employee was operating farm equipment inside the grain retaining wall to move corn seed. As the second employee was moving seed, a piece of ice on the tarp covering the pile was dislodged and slid down the tarp, striking the retaining wall and sending pieces into the air.
