An employee was using a metal pipe with 1/2-inch tines to scrape the feathers off the side of a dryer. The force of the spinning dryer caught the pipe, which impaled the employee in the upper chest and passed through his shoulder. The force then pulled out the pipe and knocked off the lid of the dryer, sending the employee to the floor. He was hospitalized.

An employee poured molten iron into a mold. The molten iron ran out of the mold at the bottom corner of the jacket and onto the employee's right foot causing a burn to the top of the foot, which required hospitalization.

An employee was walking through a doorway where a plastic parking lot boundary stake (approximately 3 feet in length) was laying on the door threshold, propping open the door. The employee stepped over the stake to exit. When she returned to re-enter the door, she went to reach for the stake to move it and fell face forward, hitting her chin and landing on the concrete floor in the vestibule. She fractured her shoulders.

On March 20, 2018, at 6:10 PM, an employee was helping to tighten a tension pulley on a ripsaw when his right hand was caught between the chain and sprocket. He experienced pain, swelling, and bleeding and was hospitalized with contusions and a broken bone that required surgery.

An employee was using a chainsaw to fell a 68-foot tall, 16-inch to 20-inch diameter at breast height chestnut tree when the tree began to fall and struck another tree. The butt of the tree kicked back, striking and fracturing the employee's right leg just above the ankle.

A horse was in a horse ring and was hooked to a carriage. The horse became spooked and started running, broke free of the reins, and attempted to exit the horse ring. An employee entered the horse ring to try and calm the horse. The horse struck the employee, who suffered a serious neck injury that required surgery, abrasions, contusions, and lacerations to the thighs/arms.

An employee was changing the force draft fan belt. He turned off the fan. Due to the inertia, the belt did not come to a complete stop and began to rotate. The employee was driving the powered industrial truck, stacking product on the top of the third pallet. The employee stacked the pallet and put the forklift in reverse. The employee then placed the forklift in forward, assuming that the forks were still elevated. The forklift fell on its side, causing the employee to break an ankle.
