Rice was not coming down to the bran remover from the auger/conveyor above. An employee removed the round plate from the square round pipe and stuck his arm inside, feeling for the cause of the choke. His hand contacted the rotating screw auger/conveyor above. He suffered an amputation of his right hand up to and including the mid-point of his ulna and the radius of his right forearm. He was hospitalized for surgery. | |||||

Worker climbed a mobile ladder-platform; he then climbed over the ladder-platform guardrails onto a chain-driven paddle conveyor. He was trying to reach a gasket on a pipe feeding the conveyor. He was wearing a hard hat, but was not wearing personal fall-arrest equipment. As he tried to reach for the pipe gasket, he lost his balance and fell more than 11 feet to the concrete floor below, landing on his back along with the pipe. Employee was installing a heavy-duty magnet in a hopper when he came too close to the side wall. His finger was caught between the magnet and wall, which resulted in an avulsion. Employee was treated at the scene and then transported to the hospital. To stitch his finger, the doctor had to shave his bone, which resulted in a partial amputation. | |||||
