This caused suspension to shift on the truck and glass panes estimated to weigh 750 lbs. fell off the truck onto the injured employee. The employee suffered a lower back fracture.

He noticed some gloves on top of a rotating sprocket above his work area. He reached his right arm to grab the gloves and his sleeve was caught in the chain, drawing his right arm into the sprocket and fracturing the radius and ulna.

On June 11, 2016, an employee was assisting the crew as it rigged out a crane. The crane operator was retracting the outrigger when the injured employee used his left hand to brace himself as he bent down to pick up the outrigger pad. He placed his left hand on the outrigger, and his finger entered the eyelets on the outrigger. As the outer section of the outrigger retracted into the inner section, the eyelet on the outer section amputated the employee's left middle fingertip.

On July 28, 2015, employees were attempting to pick up a concrete cure deck bridge. There were three employees (one in the fork lift and one on each side of the bridge holding tag-lines) performing the task. The employees attached rigging on both ends of the bridge to relocate it closer to where the pour will take place. During the pick, the rigging fell out of the hook causing one side of the bridge to fall and hit the employee fracturing the tibia/fibula.
