On September 28, 2015, at 7:00 p.m., an employee was adjusting the stroke length on a pneumatically powered cylinder. The cylinder actuated when the employee reattached the air source (100 psi) to the pneumatic cylinder, amputating the employee's right-hand pinky fingertip in the pinch point between the end of the cylinder body and the cylinder rod cap.
