Four employees were attempting to modify an approximately 10 ft. high by 10 ft. long non-assembly single piece metal scaffold system on wheels and lay it on its side in order to replace 2 x 8 wooden platforms. The scaffold kicked out while the employees were tipping it over and the scaffold fell onto an employee's left leg resulting in a compound fracture.

An employee was cleaning inside a machine when another employee started the machine. The employee suffered hand injuries, broken bones, and a deep laceration of the middle finger.

An employee was using a crane to move loads of pipe from the shop floor onto the shelving system in the warehouse. He noticed that one of the pipes in the load was stuck on the lip of the warehouse shelf. He climbed a step ladder and attempted to unhinge the pipe using a crowbar. When he used the crowbar, the load began to shift on the shelf and he jumped from the ladder to avoid it, landing on his left leg on the concrete shop floor. He fractured his left knee and tibia and was hospitalized.
