Two employees were transporting a cold box with a self-propelled modular transporter. As the load passed a power pole with a distribution transformer, the transformer arced onto a chain securing the load. The current traveled through the equipment and shocked both employees.

While climbing a ladder on a fixed platform, an employee reached for a handrail to pull himself onto a deck when he fell approximately 8 feet to the lower level, landing on his feet. The employee was hospitalized with a fractured left foot.

On October 1, 2020, an employee was assisting a subcontractor with a valve packing repair. During the repair, the valve packing dislodged creating a chlorine gas leak. The employee was evacuating the area when the hose of her supplied air system pulled her mask off, exposing her to the chlorine gas. The employee was hospitalized.

An employee was conducting morning inspections in a caustic unit. A section of CPVC pipe 15 feet above broke off as a valve was being released, fell, and hit the employee in the head. He suffered a skull fracture and lost consciousness. He was hospitalized.

A crew of carpenters were in an electric work cart near a handrail. As the driver of the cart was backing the cart up, the heel of his left foot came into contact with the rail, causing his foot to twist. The foot was broken and he was hospitalized, undergoing surgery.

An employee was removing a nozzle from a relief valve with a 48-inch pipe. He put one leg over the pipe wrench and used his body weight to attempt to break the nozzle loose. On the first push, the wrench slipped off the nozzle, which caused the employee to fall downwards with his fingers under the wrench. The force of the fall onto the concrete floor caused his left middle fingertip to mostly separate.

An employee was adding a bag of sodium thiosulfate to a storage tank containing sodium hypochlorite and a weak caustic solution. The bag fell into the tank and the liquid in the tank splashed onto the employee causing second degree burns to the right and left thigh, abdomen and left hand.

An employee was using a high-pressure sprayer to clean inside a vessel when the spray nozzle trigger became stuck. As the employee attempted to loosen it, it sprayed his left leg, cutting it.

An employee and his driving partner were disconnecting a tanker trailer from their tractor. Due to the weather conditions, the release handles on the fifth wheel coupling had ice build-up on them. The employee was disengaging the fifth wheel with a fifth wheel puller when the puller slipped. This caused the employee to lose his balance, falling backward and striking his head on an adjacent trailer. He suffered a head laceration and was hospitalized.

An employee was maneuvering a manlift over a speed bump at a crawl when the manlift accelerated and lunged forward as the front wheels went over the speed bump. The employee was lifted into the air and straight back down onto his feet in the basket. He then dropped down into a seated position. This jarring action broke his lower right leg.

An employee was loading his trailer when he heard a hissing sound. He climbed the ladder to check the dome of the trailer, tightened the bolts and began to climb down the ladder. As he attempted to put his foot down on the second rung, he lost his footing and fell to the concrete below, fracturing his skull and suffering a concussion.
