On 6/4/17, at 8:55 a.m., an employee was backing up a piece of power equipment in an aisle and turning it when his ankle was crushed between the equipment and the bumper for pallet racking. He suffered a broken leg and was hospitalized.

On 4/22/15 at 1:00PM, an employee was moving a load of bent plate steel, 10 pieces @ 200 lbs each, on a material cart. While pushing the cart over a transition in the floor (between two shops), the material shifted and caught his hand between cart and the material, crushing his left hand forefinger. The employee's finger was surgically amputated at the hospital.
