At about 5:30 a.m. on April 29, 2019, an employee reached into a machine, through a light curtain, to adjust a proximity switch. The carriage of the machine lifted up and pinned the employee against the roof of the machine. The employee suffered rib fractures and was hospitalized.

An employee was unjamming a part in a nearby machine when she stepped back into an unguarded robotic material handler. The handler caught her left leg, breaking her fibula and severely lacerating her leg from the kneecap to the ankle. She required surgery.

On July 15, 2016, an employee was skimming slag off the top of a pot of molten bronze when some of the slag and molten bronze dropped onto his coveralls. The molten bronze and slag, with a temperature over 2000 degrees Fahrenheit burned through his coveralls, seriously burning his shin, ankle, and foot before his boot could be removed.

An employee was on a 10-foot step-ladder installing some piping in a new structure. The ladder started to "walk" and began to kick out. The employee reached up to the pipes to steady himself, but then fell and broke his wrist.

On April 22, 2015, an employee was crossing over a grassy area between two facility parking lots on her way to work. As she went to step down to the parking lot, a heavy gust of wind from behind made her lose her balance. She fell down and broke her right leg at the femur.

The employee was checking the ink level on a print department machine without shutting it down. As the employee reached in to check the ink tray, the employee's shirt was pulled into a roller or shaft. The employee's left arm and shoulder were subsequently pulled into the machine. The employee was able to pull the arm free, but suffered lacerations to the left arm and injured the shoulder. The employee was transported to Freemont Medical Center and then transferred to a hospital in Omaha.
