The injured employee (mechanic) was troubleshooting a box truck and asked another employee (Employee 2) to go inside the truck. The mechanic told Employee 2 to press on the break and push on the accelerator to hear the sound the truck was making. The mechanic was under the truck when it came off the jacks and rolled forward striking the mechanic and causing rib fractures and trauma to the chest.

An employee was working on a scaffold setting up a 30" pipe for pre-heat. The pipe was tack-welded where the set-up was taking place. Workers on the other end of the pipe removed the pipe fitting dogs, and 15 minutes later the tack welds at the set-up area failed. The employee's lanyard was between the pipe and the scaffold deck at the time. When the tack weld failed, this caused the pipe to fall and pinch the employee's left knee against the pipe, cracking the fibula.

When it released, the weight shifted, pinching his left ring finger between the hopper and the forklift tine. This resulted in a crushing injury that required amputation to left ring fingertip.

An employee and a coworker were using an air-operated diaphragm pump and hose to transfer hot cooking oil from a fryer into a travel tote. The employees made the connections, turned on the air supply to the pump, and commenced pumping. After a few minutes, the discharge hose split due to the temperature of the oil (approximately 250 degrees) allowing the oil to splash the employee. He suffered third degree burns on his hands, abdomen, groin, and lower extremities.

Employee was installing steam trace lines from an aerial lift. The individual then bent over while working; the aerial lift moved, catching the employee's head between the cage of the aerial lift and an iron beam of the support structure for the unit.
