A wooden pallet got stuck on a roller conveyor. The employee hooked a chain onto the pallet and the other end onto the mast of a forklift. As the employee hooked the chain onto the mast of the forklift, the forklift operator side shifted the forklift mast, causing the employee's finger to get pinched between the hook on the chain and the forklift mast. The employee suffered a fingertip amputation of his right index finger. |
