On 7/1/15 an employee was working at the Firestone Plant in the scale house, when it was discovered that a gate on the asphalt silo had a broken bolt and was not functioning properly. At ~ 12:40 AM while attempting to replace the bolt, his crowbar slipped and struck him in the face. He was admitted for in-patient hospitalization for a fractured bone above the eye (no eye loss/damage) and a fractured cheek bone.
