On March 18, 2021, at approximately at 3:30 PM, five employees were lifting a car transmission on a pallet off of a truck on a rainy day. While lifting the pallet, the transmission slid and the five employees quickly set the pallet on the truck again and the injured employee's finger was trapped between the truck and the pallet resulting in a fingertip amputation.

On September 22, 2020, an employee was moving a new log splitter when it struck a pallet on the floor, causing the arm to come down on the employee's hand. The employee suffered an amputation of their middle finger. Their ring finger was able to be reattached.

The guillotine was guarded at the time of the incident.

On October 13, 2017, an employee was untangling steel tubing on the front load end of a plating line when the machine recognized a change in tension and began pulling the tubing, which then pinched and amputated the employee's right ring fingertip. On April 13, 2017, an employee suffered a fracture of his right femur after he was struck by a displaced railroad rail while performing cutting operations. The rail was hoisted via backhoe grapple while being cut by the injured employee. As the employee was making the cut the rail fractured, striking the employee in the leg.

On 12/2/2015, at approximately 12 p.m., an employee was climbing a ladder to the roof to install fall protection anchor points. The decking under the ladder was frosted over. As the employee climbed the ladder, the ladder slipped, causing the employee to fall approximately 10 feet to the decking. The employee was hospitalized with contusions to the left shoulder, left leg, and ribs.
