On 9/20/2021, around 6:50 a.m., an employee was taking grain samples when a splice on the conveyor belt pulled the sampling device out of the employee's hand. The employee's hand made contact with the sampling device and the conveyor cover. The employee sustained amputations to two fingertips on their right hand and was hospitalized.

An employee was conducting sheet decking on a roof that was approximately 10 to 14 feet from a lower level when he slipped and fell to the concrete floor below. He fractured his sacral area, right wrist, right elbow, and L1-L2 and L4-L5 vertebrae, requiring hospitalization. The employee was not tied off at the time.
