On November 11, 2021, an employee was performing die setup on a hydraulic press. The employee loaded dies into the press and actuated it to verify proper die alignment. The upper and lower dies made contact resulting in die failure. Fragments of the die struck the employee's hand causing a severe laceration and fracture to their index finger. Work in a 4-foot pit had just been completed and the diamond floor plating over the top of the pit was replaced incorrectly. As the injured employee was walking to check the valve, he fell through the floor plating over the pit. The injured employee sustained a right leg laceration on the corner of the plate.
