The employee was trapped underneath and pinned to the ground. The employee suffered six fractures to his pelvis and was hospitalized for surgery.

A log was being loaded onto a truck with a grapple. Because of the log's size, the grapple operator could not see the back of the truck and needed assistance placing the log in the truck. An employee climbed onto the back of the truck in order to assist. The log started rotating as it was being placed onto the truck. When the employee jumped out of the way, their hand was pinched between the log and a part on the back of the truck. The employee's left pinky finger was surgically amputated.

An employee had been conducting flagging operations at an intersection where other employees were applying hot tar to the road. The employee went to get onto the hot tar buggy to ride to the next intersection but fell off the buggy and landed in hot tar that had puddled on the road. The employee sustained burns to his left arm and right hand. He also sustained a bruised left leg.

The employee suffered a puncture wound on his right forearm and a broken finger on his right hand. He was hospitalized for treatment.
