On 2/19/2021, an employee was near the top of an extension ladder attempting to "blow out" a clogged milk line. There was ice on the floor where the ladder was standing. The ladder slipped on the ice, and the employee fell onto products/equipment, a concrete barrier pole, and the floor. The employee was hospitalized with fractures to the left tibia, nose, left side ribs, and knee cap.

An employee was working with a co-worker on a brake press bending stainless steel when he noticed one piece had not bent at a 90-degree angle. He went to hit it again at a small place in the middle, and when doing so, the machine crushed his fingers. The employee suffered a ring fingertip amputation and amputation of his middle finger.

An employee was using a pallet jack to unload boxes of furniture from a trailer. A box of furniture was sitting on a pallet that was partially shattered. As the pallet was pulled, it completely shattered causing the furniture to fall onto the employee. The employee suffered displaced fractures to both bones in the right ankle.

An employee was hot air welding PVC roof seams and transitioning to a new location on a roof when his extension cord bumped a gallon container of acetone. The container began to tip and he tried to catch it with his right hand. Some contents spilled and were subsequently ignited by a flameless hot air welder that was approximately 2 feet away. His pants and shirt caught fire, and he received second degree burns to his right hand, arm, and bicep. He was hospitalized.

An employee was guiding a chiller head as it was being hoisted by a chain hoist. A strap that was used to hang the chain hoist snapped, causing the chain hoist and the chiller head to fall. As it was falling the chiller head hit a section of steam piping and veered off towards the employee, landing on the bridge of his right foot. Four metatarsals were each fractured in two places, requiring hospitalization.

An employee was setting up to cut a cable and needed to release a come-a-long that was attached to the temporary suspender at the cable. At this point, the temporary suspender spun and pinched the employee between the existing suspender and temporary suspender assembly. The employee suffered a broken left tibia and fibula, dislocated right knee, and cut head.

An employee was working outside around noon, putting up a satellite dish outside a house, when he started feeling dizzy and nauseated. He came off his ladder to stand on the ground and was walking to a van to sit in the shade and drink water when he passed out. He was hospitalized for possible dehydration.

While demolishing an overhead door, an employee loosened bolts to the bracket holding the spring that helps lift the overhead door. The spring was un-sprung and still had tension. When the bolts were removed, the spring sprung, causing the bracket to spin and strike the employee. The employee suffered left arm lacerations, left thumb and finger fractures, a broken right arm and thumb, and multiple right arm lacerations.

He then shut off the motor and was wiping oil off the machine when a blade grabbed his rag and pulled his hand into the cutting head.
