The employee was hospitalized with fractures to the left tibia, nose, left side ribs, and knee cap.

An employee was working with a co-worker on a brake press bending stainless steel when he noticed one piece had not bent at a 90-degree angle. He went to hit it again at a small place in the middle, and when doing so, the machine crushed his fingers. The employee suffered a ring fingertip amputation and amputation of his middle finger.

An employee was using a pallet jack to unload boxes of furniture from a trailer. A box of furniture was sitting on a pallet that was partially shattered. As the pallet was pulled, it completely shattered causing the furniture to fall onto the employee. The employee suffered displaced fractures to both bones in the right ankle.

An employee was hot air welding PVC roof seams and transitioning to a new location on a roof when his extension cord bumped a gallon container of acetone. The container began to tip and he tried to catch it with his right hand. Some contents spilled and were subsequently ignited by a flameless hot air welder that was approximately 2 feet away. His pants and shirt caught fire, and he received second degree burns to his right hand, arm, and bicep. He was hospitalized.

As it was falling the chiller head hit a section of steam piping and veered off towards the employee, landing on the bridge of his right foot. Four metatarsals were each fractured in two places, requiring hospitalization.

At this point, the temporary suspender spun and pinched the employee between the existing suspender and temporary suspender assembly.
