An employee was helping to clean up around a conveyor belt area. Another employee turned the conveyor on so it could move and clear some material. The employee attempted to remove a large piece of bark under the conveyor. His left arm was caught in the moving parts under the conveyor. He was hospitalized with a broken left arm and required surgery.

An employee was aligning/installing the bottom punch in a forging press. An airline was hooked up to extend cylinders for the die pod. The airline became disconnected and the die pod fell, pinching the employee's left index finger between the die and the punch. The finger was amputated to the second knuckle.

The employee slipped on the incline that led up to the conveyor, fell down, and his foot got caught on the conveyor. The conveyor belt pulled off his boot and cut his foot. The employee was hospitalized.

In the planner area, an employee was piling lumber that was being transported by a lumber chain. One of the chains became jammed. The employee reached between the chain way because the chains were doubled over. Energy was stored in the gear box due to the jam. The employee manually tugged the belt associated with the belt/pulley mechanism for the 2 hp motor and gear box. The motor/gear box drives the chains. When the employee tugged on the belt, it released the stored energy from the jammed
