At 11:45 a.m., on January 24, 2019, two employees were inspecting the feed throat of the barrel in an injection molding machine that was having a material flow issue. The screw was retracted and overheated degraded nylon was forced out through the screw feed throat, burning one of the employees on the hands and face. Personal protective equipment was not worn at the time.

He attempted to stop the vehicle with his foot, but it was crushed between the truck and wall. His foot/leg suffered multiple fractures, requiring hospitalization and surgery.
