Employees were attempting to move a crate of six granite slabs (weighing approximately 800 pounds each) from the back of a trailer using a chain. One end of the chain was tied to the mast of a forklift and the other wrapped around the bottom of the crate. Before they pulled the crate from the trailer, a strap that was holding the slabs broke, allowing the slabs to fall onto the injured employee, who broke both legs.

Employees were uprighting a steel beam while constructing a gas station frame. A beam was being hoisted when one edge became stuck on the dunnage. When the beam swung loose, it struck an employee, causing the employee to be caught between the hoisted beam and a beam on the ground. The employee suffered a compound fracture to the left leg.

An employee exited the building to go to the back patio and the door locked behind him. To re-enter the building, he attempted to climb a security fence. The employee became caught up in the fence's barbed wire and fell to the ground, landing on his back and right side. Worker was hit on the left forearm by a backhoe's bucket while he was standing in a 3 feet deep trench parallel to another trench approximately 4 feet away. The backhoe was approximately 8 feet away from the injured worker. The dirt in the trench where the backhoe was located gave out and caused the backhoe to swing and hit the employee.
