An employee was preparing to unload the contents of a tanker. He climbed the tanker ladder to cut off the seal and loosen wing nuts to allow the tanker to pump off. An employee was performing maintenance on a wheel loader (front-end loader) and removed an access plate to the bottom of the engine compartment. After the fasteners were removed, the access plate fell to the floor and crushed the employee's forearm between the plate and the floor. The employee suffered damage to the muscle and connective tissue (no broken bones).

An employee standing on an 8-foot ladder was using a hammer to remove wooden blocks wedged between the door and the wall. He fell to the ground and fractured his lower leg.
