| On August 13, 2015, an employee was operating an overhead crane to move a steel beam that was approximately 4' by 15' and weighed 4,000 pounds. The beam was sitting in the I position and the employee was using beam clamps to move the beam to horses. The employee's right middle finger got caught between the beam clamp and the beam flange.
