Two employees were operating a 300-ton press. The injured employee was stacking manufactured parts from the mechanical press. As the injured employee was removing a cut part from the press, the press activated and his arm was crushed in the press. The employee's arm was amputated at mid forearm.

An employee was working on an 8-foot ladder and fell backwards from the ladder onto the concrete and striking his head on the threshold of a doorway. The employee sustained a head contusion and laceration.

An employee was attempting to weld a piece of sheet metal to another assembly. The tack weld broke and the sheet metal fell onto the employee possibly fracturing the right knee, left ankle and heel.

Employee was fabricating sound proof enclosures and began not feeling well. He was given water and told to sit down and rest. The employee became sick to his stomach and complained of chest pain. The employee was hospitalized for dehydration.

A machine drops dough into a hopper and then onto a conveyor. The machine operator knelt down on the ground and his hand went under the conveyor. His hand activated a sensor that signals the dough cutter to come down and cut/section the dough. The dough cutter, which is not a blade but a chunter, pinched the thumb on his left hand.

The employee was cleaning the windshield of a semi-truck with the hood of the truck open. He finished cleaning the windshield and went to step down when his wedding ring caught on the hood. The weight of the employee's body and the ring being caught on the hood caused the hood to cut the employee's finger. The finger was later amputated at the hospital.
