A Tyson Poultry commercial motor vehicle driver was struck from behind by an empty cage being carried on a three-wheel forklift. The driver suffered lacerations to the right elbow and the top of the left hand, a compound fracture to the right tibia and fibula, fractures to three lower-back vertebrae, and body abrasions.

An employee was walking in the rain and observed an aluminum container on the ground. The employee picked it up and attempted to see if there was anything in the container. The top came off and exposed the pellets inside to the rain. The pellets inside the container were highly reactive to water and exploded when the rain entered the container. The employee suffered burns to the eyes.

An overhead lineman was elevated in a bucket while removing the rubber cover from a primary power line. An arc flash caused first- and second-degree burns to the employee's stomach and the area under the right arm. The employee was hospitalized.

An employee was disassembling gear boxes. As the employee reached under a load that was lifted by a crane, a bearing fell out and landed on his left hand causing fractures to two fingers. The employee was hospitalized.
