An employee was helping a truck driver unload a truck at the post office dock. As the dock lever pulley was engaged, the front lip of the dock leveler did not extend properly. The employee attempted to lift the front lip and her right index finger entered the hinge where the lip and plate meet. Her finger got caught and her right index fingertip was amputated. |
