On or about April 10, 2015, at approximately 1300 hours, an employee was hospitalized with two fractures to his distal tibia and fibula. He was assigned to a four-man team to repair a compressor located at an Oxy-Permian gas facility when he used a golf cart to retrieve equipment at another compressor. He hit a barrier with the golf cart, pinning his leg between the cart and the barrier. He was injured and subsequently hospitalized. | |||||

Employees were hanging a horse head on a well. The injured employee was on a pumping unit, waiting for the horse head to be lifted with rig blocks, with a safety retractable line attached. The line got caught in the blocks and pulled the employee off of the pumping unit, causing the employee to fall 10 to 15 feet before the safety line engaged.
