| On or about April 10, 2015, at approximately 1300 hours, an employee was hospitalized with two fractures to his distal tibia and fibula. He was assigned to a four-man team to repair a compressor located at an Oxy-Permian gas facility when he used a golf cart to retrieve equipment at another compressor. | Employees were hanging a horse head on a well. The injured employee was on a pumping unit, waiting for the horse head to be lifted with rig blocks, with a safety retractable line attached.
