On September 19, 2015, at approximately 11:00am, a millwright was injured while spot welding a hard hydraulic line on a forklift. The line was under pressure and hydraulic fluid was not removed from the line. The line ruptured and the millwright received 2nd degree burns to the top of his head and was hospitalized.

An employee was working on a "Right of Way" on 6/8/2015. He forgot an instrument and got on an ATV to go back for it. While passing the "Right of Way" on a switchback, he hit something (presumably a rock or waterbar) and was thrown from the ATV at approximately 9 a.m. He suffered injuries to the shoulder blades, collarbone, and three ribs (broken) and required hospitalization.

On March 29, 2015, at approximately 3:00 p.m., an employee sustained an injury to the middle finger and index finger on his right hand, resulting in amputation. The site port cover was removed from the door-facing press to blow out the fiber incline conveyor. The conveyor was not shut down at the time. An air wand got caught between the conveyor and tail roller, and the employee's hand was pulled into the framework between the air wand and the conveyor housing.
