He lost his balance and fell into the hopper, landing on another employee (who had been feeding the hose up to him). The employee who fell suffered a closed head injury; the employee he landed on suffered a shoulder injury and a broken collarbone.

Two employees were removing a valve from a 4-inch hydrogen line in a sulfur unit. One employee was loosening the bottom bolts of the flange. As the flange was opened, hydrogen escaped and caused a flash fire that burned the two employees. The employees suffered second degree burns on the ears, right wrist, right arm, back of the neck, and the back. One employee was hospitalized.
