An employee was loading peanut stock onto a scale from a hopper. An inspection cover was removed and his hand was inside an inspection-hole opening when another employee started the fan. The fan blade then partially amputated his right middle finger.

An employee was responsible for loading rail cars with peanuts. The employee climbed to the top of the rail car to position the shoot for the loading process. The employee was wearing a retractable harness at the time of the incident. The train began moving forward at a slow rate of speed, causing the retractable lanyard to become fully extended. The employee was pulled from the top of the rail car and impacted the ground resulting in serious injury (bleeding on the brain and a shoulder injury)

An employee was standing on a fixed metal ladder at the pivot point on an irrigation system while using an electrical tester to determine if power was getting to the junction box. He incurred injuries from an electrical shock, which caused him to fall off the ladder. He also received head injuries from the fall.
