An employee was using a tool to tighten a bolt to a flange. As the employee was removing the tool, the tool actuated resulting in the amputation of the left index fingertip.

On January 29, 2018, an employee was performing maintenance on equipment when he slipped and fell. His left hand then then went between a belt and pulley on an adjacent machine, amputating his left middle fingertip. The belt and pulley was unguarded at the time.

Drilling pipe was being rolled off of a truck onto the rack. The pipe kicked back as it reached the bottom, catching an employee's right thumb between the pipe and the stack of pipes. The employee sustained an amputation to half of his thumb tip.
