An employee was removing a hook from an outfeed table that pulls tubes through the annealing furnace. The hook did not come off the chain and table on its own. The employee noticed the hook still attached and went to unhook the table. The hook and chain were still moving. The employee's right finger was smashed between the hook and chain and the fingertip was amputated.

An employee was laying decking material for a reinforced concrete roof. The employee was not able to get decking material under a piece of bent rebar. The employee was attempting to bend the rebar up when it suddenly became free from the cinder block wall, causing the employee to lose balance at the leading edge and fall approximately 10 feet to the floor. The employee fractured the left wrist and femur.

An employee was standing on some equipment (24" from ground level) spraying down parts when he lost his footing. He reached out to catch himself when his right-hand pinky finger hit the sharp metal blade on a machine that cuts the feet off of chicken. The joint where his pinky finger and hand meet was dislocated and the blade cut through the area, causing a severe laceration. The machine was locked out at the time of the incident. He was hospitalized and had surgery.

An employee turned off an auger at the bottom of a ribbon blender to find pieces of a plastic bucket lift that had broken and fallen into the ribbon blender. The employee went around and had opened the drain cover of the auger to retrieve the pieces when the auger activated and amputated the employee's left thumb. The auger was not locked out/tagged out at the time.

An employee was on the back of a trailer that was in the dock doorway. As he was filling an air bag, a coworker driving a yard truck pulled the trailer away from the dock and the employee fell out of the trailer. He dislocated his right elbow.
