| An employee was pulling the die from a rack when the push/pull mechanism of the die cart failed. When the employee realized this, he removed the chains from around the die and proceeded to set the brake, then pushed the die onto the die cart from behind the rack. The die cart then moved backwards, causing the die to start to tip and fall.
