The tension from the come-along cable ratchet system caused the hook to fly back toward the employee, striking and injuring his left thumb resulting in medical amputation of the thumb tip.

He fell through a 4-foot by 7-foot opening whose plywood cover slid, landing on the concrete floor about 12 feet below and fracturing both heels. He was hospitalized.

The forklift rolled forward crushing his legs between the bumper of the cargo van and the material on the forklift causing fractures to both legs. The employee was hospitalized.

He was trying to remove the raw material skeleton blank from the die when the press cycled, causing the die to close on his hand. The die came to a stop at the bottom of the stroke, trapping his hand in the die and severing his left middle fingertip.

As the employees moved toward the drain, the Green Liquor flowed around behind them and rose above their boots. The Green Liquor seeped through
