An employee was walking on a shanking conveyor when he slipped while stepping over a piece of conduit. His hand went into a chain and sprocket on the roller conveyor, resulting in an amputation to his right ring fingertip. The chain and sprocket were unguarded at the time of the incident.

An employee was freeing a jammed wood product (veneer panel plywood) from a saw line workstation. While freeing the jammed plywood from the grader bin, the operator pinned/pinched his arm between a piece of metal angle iron (under a load) and the metal cross member causing a puncture wound on his right arm below the bicep.

An employee was adjusting the feed roller to the spec saw. The employee's watchband was caught in the keyway and the employee's hand was caught between a driveshaft and the feed roller resulting in tendon damage to the left hand and a laceration that required hospitalization.
