An employee was in the trench putting a gasket on the previous pipe joint when an excavator started lifting the cut joint. The pipe swung like a pendulum as it was lifted and smashed the employee's left-hand fingers between the swinging pipe and the existing pipe. Two fingers were surgically amputated to the first knuckle.

The employee was sitting at the edge of a large commercial dumpster with his feet inside of the receptacle. The employee was attempting to reach for a piece of plywood, lost his balance, and fell backward, approximately 8 feet to the dirt ground below. The employee was hospitalized with fractures to the neck.
