A maintenance employee was removing a level control valve when he was sprayed with Hexamethylenediamine (HMD). The contents of the pipe were under pressure and the liquid was hot, but less than 100 degrees Celsius. The employee sustained chemical burns and possibly thermal burns to his face and back.

A piece of quartz countertop material was stored on an A-frame metal storage stand that was 9 3/4 inches off the floor, with wheels on one end and permanent legs on the opposite end. The material slipped off the edge of the stand onto an employee's right foot, breaking two toes. The employee was admitted to the hospital.

On May 31, 2019, an employee was clearing a jammed bag from the bagging machine sealer when his right middle and ring fingers were caught between the transfer belt and the bagging machine's shaft. He sustained a laceration to both his right middle and ring fingers and was hospitalized.

An employee was starting work on a precipitator and was using a sleever bar to remove the door, which was 18" x 18". He removed the bolts and some of the silicone from the door, but the door would not open. He went to the front of the equipment, removed the inspection window, and used the sleever bar to reach inside, at which point he was electric shocked. The electrical current went in his left ring finger and out through his right foot.

An employee was on the ground floor securing the containment with flexible tarp when the wind started to move it around. A gust of wind blew against the flexible tarp, knocking him backwards and causing him to land on the ground on his right wrist. His wrist fractured and required hospitalization.
