An employee was placing a rug into the mandrel opening to be rolled and wrapped when she experienced an issue getting the rug corner into the slot opening. At the same time, a co-worker came to her left to place a second rug into position to be rolled and wrapped on the same cycle. The switch that clamps the ends of the rugs was activated and the employee's right middle fingertip was pinched, resulting in a partial amputation.

An employee was emptying cardboard from a baler. As he was opening the baler door, he noticed it was stuck on the top gate. He lifted on the top gate to free the bottom door and the door swung open, smashing his right-hand fingers against the door latch. His right ring fingertip was amputated.
